For your first appointment
- Referral letter from your doctor, dentist or orthodontist.
- Existing X-rays, CT or MRI and reports.
- Medication list, allergies and relevant medical history.
- Contact details for professionals involved in your care.
- For a child, a legal guardian and the necessary documents.
Understanding the decision
Diagnosis, options, expected benefit, alternatives, risks and recovery should be understood before consent. Mention travel, scheduling and follow-up constraints.
Protecting your health information
Do not send identifiable X-rays, reports or medical details through an unsecured form or personal email. The secretary will explain the appropriate secure route when documents are needed.
